Summary

Idea document-link and error underlines must cover the same UTF-16 range as the
syntax token ( Langium `$refNode` / semantic token ), including when non-ASCII text
appears earlier in the file.
Native extract spans are UTF-8 byte offsets
( local_symbolic_analysis ); LSP `Position.character`
is UTF-16. Convert before `positionAt` so the underline does not drift off the token.
