Indexing should be triggered by manual indexing commands ( Refresh / Clear & rebuild ).
Refresh and Clear & rebuild always rediscover bases first via
refresh_rediscovers_bases /
base-registry.ts `refresh`, then soft-sync or
rebuild the active base.
