Table `documents` tracks which files the indexer has seen.
Columns:
- `file_uri` TEXT PRIMARY KEY — base-relative path
- `content_hash` TEXT NOT NULL
- `indexed_at` TEXT NOT NULL
- `mtime_ms` REAL ( schema v3 ) — soft sync skip when FS mtime matches
Soft sync and idle stale checks use this table
( soft_sync, idle_stale_check ).
